Case description

A 70-year-old patient presented with acute severe cholangitis and acute kidney failure. Ultrasound imaging revealed multiple gallbladder stones and stones in the lower common bile duct (CBD). An emergency ERCP was performed with the following findings and outcomes:

- Access to the CBD was achieved with ease; however, no stones were initially identified within the duct.

- Contrast injection revealed a lower cystic duct insertion with stones causing compression of the lower CBD, consistent with Mirizzi Syndrome.

- Large stones were located within the cystic duct. A guidewire was successfully advanced into the cystic duct, followed by large balloon dilation.

- Two large stones were extracted successfully, accompanied by the release of pus.
