Case description

Video presented above details a colonoscopy of a middle aged woman with a large pedunculated polyp in the sigmoid colon. The head of the polip was obstructing the lumen. Also, the patient had many comorbidities, such as renal failure, morbid obesity and antithrombotic therapy.

Each step of the procedure was shown from the beginning to the end without edits.
Step 1: Epinephrine injection into the head and stalk and base to create vasospasm and hemostasis.
Step 2: Endoscopic loop ligation
Step 3: Snare resection with coagulation current
Step 4: Retrieve the specimen and pin the stalk for pathology interpretation (as stalks can shrink); it helps pathologist slice the polyp parallel to the stalk.
Step 5: Clip the resection base above the loop for additional hemostasis.
