Case description

This video shows a greenlight XPS prostate surgery for the treatment of male BPH performed by Dr. Kevin Zorn.

In step 6, we perform a similar approach to what was carried out during the 4th step of standardized PVP Greenlight. In short, we rotate the 23F Storz sheath to the 3'- o'clock position so as to allow the fibre to exit near the 9'-o'clock area on the screen. Using a lowered 120W power, we will incise through the adenoma at the bladder neck to carefully cut down to the surgical capsule. Once this is landmark is identified at the 11'-o'clock area, the groove is carried out back to the apex of the prostate. The angle of the laser beam and energy is also used tangential to the capsule to create a clean, enucleation like plane. This ensures complete adenoma resection and minimization of residual necrotic tissue which delays wound healing. This move once again detaches the adenoma off the capsule to allow powerful 180W vaporization and resection of the right lateral lobe. Once detached from 11'- oclock to the 9'-o'clock area, the 7'-o'clock grove is connected at the apex to release the tissue off the sphicter to minimize the risk of incontinence.
