Case description

Surgical case: axilloscopic lymph node dissection. A valid de-escalation concept minimizing scarring and reducing morbidity within the context of modern oncoplastic breast cancer surgery. A 50 y old female patient presented with paget's disease of the nipple associated with central (retroareolar) 2cm mass. Core biopsy revealed invasive ductal carcinoma grade II. Clinicoradiological assessment demonstrated positive axillary lymph nodes with metastatic involvement confirmed by FNAB. The case was staged as T2N1M0, hormone receptor (HR) positive and HER 2 negative, mandating axillary lymph node dissection (ALND) within the context of the grisotti flap oncoplastic procedure.
