Case description

A 50-year-old patient with a history of malignant neoplasm of the distal rectum, 1 cm above the dentate line, Rullier type 2, stage T3N2M0, was admitted for neoadjuvant therapy with radiotherapy and oral capecitabine. There was no complete clinical response, so the patient underwent surgery for total laparoscopic radioproctectomy with coloanal anastomosis without a safety ileostomy. The pathology report was: ypT2 ypN1.
