Case description

An elderly patient with a history of two episodes of mild biliary acute pancreatitis and a hepatic cyst, suspected to be hydatid on ultrasound with a negative Western blot. The patient had a history of blunt abdominal trauma requiring laparotomy via a right paramedian incision 20 years prior to the scheduled surgery. During pneumoperitoneum created through the umbilicus, a small bowel injury was noted, which was sutured, and the incision was enlarged to examine for other possible injuries. Once this stage was completed, the scheduled surgery continued. The procedure included hepatic pericystectomy, cholecystectomy, and omentoplasty with drainage performed laparoscopically.
