Case description

This video presents a robotic cholecystectomy in a patient with acute cholecystitis and a 48-hour clinical history. There were no previous biliary symptoms, no cholestasis, and liver enzymes were within normal limits. Imaging with ultrasound and CT confirmed infundibular macro-lithiasis and inflammatory changes. The procedure was performed using four robotic arms without an assistant trocar. Indocyanine green fluorescence cholangiography and intraoperative cholangiography were both employed to assess the biliary anatomy. The critical view of safety was achieved under ICG guidance. Dissection was carried out using robotic scissors, offering precise and layered tissue handling. Unlike the monopolar hook, which reflects a laparoscopic legacy, scissors enable a more controlled and anatomically respectful approach, particularly valuable in acute settings.

The case illustrates stable dissection conditions and clear exposure throughout the procedure.
