Case description

Totalis Situs inversus (SIT) is an extremely rare condition where the major organs are reversed or mirrored from their usual positions. There are few reports in literature describing thoracoscopic major surgery in patients with this unusual anomaly. Here we present a video of a 48 year old female with 2 small early stage lung cancer lesions located in apicoposterior segment of right upper lobe and dorsal segment of right lower lobe. The patient underwent an uniportal approach through a 3cm incision and 2 combined anatomic segmentectomies were performed in the upper and lower lobe. The postoperative course was uneventful.
This is the first video reported showing an anatomic segmentectomy in a patient with a situs inversus condition.
