Case description

A 37-year-old female patient with a history of two vaginal deliveries presented with a one-year history of pelvic pressure and rectal discomfort. Imaging revealed a 6 cm FIGO type V intramural fibroid located on the posterior uterine wall, exerting pressure on the rectum and contributing to the reported symptoms.

A vNOTES (vaginal natural orifice transluminal endoscopic surgery) myomectomy was performed. The myometrial defect was closed in two layers using (*brand hidden*) suture material. Intraoperative blood loss was estimated at 50 mL. The postoperative course was uneventful, and the patient was discharged home on the first postoperative day
