Case description

The film presents a laparoscopic repair of an incisional hernia in a woman after numerous laparoscopic bariatric surgeries. Previously, in the left hypochondrium, a gastric band port has been inserted, which after a few years was removed and a hernia developed in this location. The hernia orifice is located lateral to the semilunar line within the oblique and transverse muscles. Placing an IPOM mesh in this location is unfavorable due to the ribs and diaphragm (despite of other disadvantages of this technique). Open anterior dissection may increase further complications (seromas in onlay or extensive dissection through a narrow orifice in sublay techniques). For this reason, the lateral TARM (transabdominal retromuscular) method was used, analogously to the TAPP method for inguinal hernia.
