Video showing a very rare case of a tumor at the entrance of the middle lobe bronchus reaching bronchus intermedius. An uniportal VATS approach was performed to perform a middle lobectomy and bronchial sleeve resection (anastomosis of bronchus intermedius to lower lobe bronchus). A tourniquets were used to retract the pulmonary artery and have a better exposure for bronchial suture. The postoperative course of the patient was uneventful
