Distended GB and CBD with Varicosities

Case description

42 yrs old female presented with severe agonizing pain in the right hypochondrium with tender palpable GB. Typical presentation of acute cholecystitis. Because she was diabetic, I decided to operate upon early. The abdominal ultrasound revealed normal caliber of the biliary tree. The labs were normal, including the bilirubin. Surprisingly I found the GB and CBD so distended with thin friable wall, and varicosities of the veins were also encountered around the porta hepatis and the GB which add difficulty to our procedure and some bleeding. I want to ask you my professors and colleague for your opinion?
