Mitral surgery may be performed either by sternotomy or by video-assisted right mini-thoracotomy when the mitral procedure is isolated. This video shows the both main techniques for mitral valve repair. 1) The classical quadrangular resection, indicated when the prolapse is limited, as in this case with a prolapse of the posterior leaflet. 2) The neochordae technique (artificial chordae with Gore-Tex®) when the prolapse is very large and preclude resection. This latter technique is also feasible for an anterior prolapse or a bileaflet prolapse. With the advent of video-surgery this technique is more and more used.
