Case description

In this experimental video I show my method of total reconstruction of the mitral valve with fixed autologous pericardium.
The intercommissural distance is measured after the native valve excision and the valve parameters are determined based on the normal mitral valve morphology. The determined parameters are marked on the fixed autologous pericardium and the new valve is tailored. The chordae are sutured to the pupillary muscles first and the leaflets are sutured to the fibrous annulus. The continuity of the posterior leaflet is restored. The new valve is tested using hydraulic probe.
Since the new valve tailoring is not easy and takes a long time, the mold of different sizes (from 28 mm to 40 mm) should be prepared to make an intra-operative new valve tailoring easy, fast and reproducible. We developed the design patent of such molds.
This method may become a very good alternative for the prosthetic valve replacement in cases when the mitral valve repair is not feasible.
