In this video we show a case of an endobronchial adenocarcinoma located at the entrance of right main bronchus in a patient with previous history of tuberculosis (strong adhesions). An uniportal VATS approach was performed through the 4th intercostal space, the azygos vein was transected and the carina and left main bronchus mobilized and exposed. The tumor was resected with partial resection of right main bronchus (free margins) and an a running suture was conducted.
