Case description

This video shows 4 different ways to transect the bronchus during uniportal videothoracoscopic left upper lobectomy. In cases of Left upper lobectomies with exposure and division of the lingular artery in the fissure the procedure is easy. After lingular branch division, the insertion of staplers for the upper lobe bronchus is safer and the risk of vascular lesion or parenchymal injury to the lingula is minimized. When the upper lobectomy is performed by using the fissureless technique, care must be taken during dissection of the bronchus in order to avoid injury to the lingular and/or lower pulmonary artery, which is usually behind the bronchus. Initial blunt dissection on the pulmonary artery separates the bronchus and the artery.
