Case description

Background: Number of ports required for the laparoscopic surgery was reduced lately. Single Port Access is an advanced procedure using only one incision. We performed SPA method to a GE junction intervention. A small incision was made in the umbilical region. The patient, complaining of upper gastrointerstinal bleeding, was diagnosed with a 3 cm x 3 cm stromal tumor. This is a report of the first SPA procedure to remove GIST tumor. Intervention: Through an umbilical incision of 18 mm diameter a laparoscopic trocar was inserted. Skin hatchs were made laterally to the first incision to permit placement of two other trocars. First, the port was created, then, excision of the tumor was performed. The upper endoscopy was of great help (intraluminal evaluation) and it was made by a collaborating gastroenterolgy specialist. The tumor was excised totally, and without any complications. Conclusion: On the video is was showed our first stromal tumor excision with a NOTES/SPA technique. The basis of minimally invasive and SPA resection of the GIST are the same. Authors: Andrew S. Wu, MD, Nithin Karanth, MD, Paul G. Curcillo II, MD
