Case description

Sarcoidosis with hepatic & visceral Involvement

Hepatic sarcoidosis is one of the extra-pulmonary manifestations of sarcoidosis, in which non-caseating granulomas form in the liver tissue.

Clinical manifestations

Most patients are asymptomatic. Symptomatic patients may present with:

- Elevated liver tests (the most common finding)

- Increased alkaline phosphatase (ALP)

- Increased GGT

- Mild elevations in AST/ALT

- Hepatomegaly

- Nodular liver lesions

- Fatigue, loss of appetite

- Pruritus, jaundice (in cholestatic cases)

Rare but severe complications:

- Portal hypertension

- Sarcoidosis-related cirrhosis

- Obstructive liver failure
