Case description

During laparoscopic cholecystectomy, the gallbladder bed is a potential source of postoperative bleeding. To prevent delayed hemorrhage or hemoperitoneum, meticulous hemostasis is crucial. Bleeding can occur intraoperatively or postoperatively due to incomplete coagulation or hemostatic seal slippage. During dissection, identify and clip small arterial branches, often in the gallbladder fossa' sub-serosal plane, that exhibit pulsation or persistent oozing. Avoid excessive electrocautery near the liver capsule to minimize bile leaks or thermal injury. After gallbladder removal, temporarily lower pneumoperitoneum pressure (~8 mmHg) to reveal and address any occult bleeding in the gallbladder bed.
