Case description

Introduction: The technique of placing an implantable port under local anesthesia has the advantage of being simple, rapid, effective, reproducible and devoid of risk of serious intraoperative complications (arterial puncture, hemorrhage, pneumothorax). On the other hand, it is a purely surgical technique. This video describes the different steps of the implantable port placement technique by stripping the cephalic vein and will be useful to all surgical residents in training and young surgeons.Anatomical reminder:The cephalic vein belongs to the superficial network of veins of the upper limb.Anatomical landmarks and marking the incisionLocal anesthesia is performed by cutaneous and subcutaneous infiltration with Lidocaine 2%.Arciform skin incision looking at the deltopectoral grooveDissection of the cephalic vein: The vein is dissectedA venotomy.Placement of the catheterChecking the position of the catheter: an intraoperative scopic checkChamber site preparationConnecting the catheter to the chamberChamber check: Aspiration punctureRinse with saline 9%Fixing the chamberSubcutaneous and cutaneous closure.A frontal x-ray: checking the correct positioning of the catheter.
