66-year-old female patient (BMI 26.4) with rectal adenocarcinoma. She underwent neoadjuvant long-course radio-chemotherapy. Preoperative CT revealed liver metastasis in segment 7. The patient was scheduled for TME and segment 7 metastasectomy. However, intraoperatively two additional lesions were found in segments 2/3. Therefore a decision was made on a two-stage operation (rectum first). The video shows TME procedure only.
