Case description

Beating-heart paravalvular leak (PVL) repair is an advanced surgical technique used to correct regurgitation occurring around the sewing ring of a prosthetic valve without the need for cardioplegic arrest. By operating on a beating heart - often with the aid of cardiopulmonary bypass and careful ventricular venting - the surgeon avoids repeated cross-clamping and minimizes ischemia-reperfusion injury, which is particularly valuable in high-risk or reoperative patients. Direct visualization of the defect allows precise identification of the leakage tract, followed by closure with pledgeted sutures or patch reinforcement while maintaining native hemodynamics. This approach reduces operative trauma, shortens recovery time, and lowers the risk of prosthetic valve damage compared with full valve replacement. Beating-heart PVL repair is increasingly recognized as a durable, safe, and elegant solution for selected patients with symptomatic paravalvular regurgitation.
