Case description

A cardiac hydatid cyst is a rare but potentially life-threatening manifestation of echinococcal infection, most commonly caused by Echinococcus granulosus. Although the liver and lungs are the usual sites, cardiac involvement occurs in less than 2% of cases, with the left ventricle being the most frequent location due to its rich blood supply. The cyst grows slowly and may remain clinically silent for years, but enlargement can lead to chest pain, arrhythmias, conduction abnormalities, embolization, pericardial effusion, or even sudden death if rupture occurs. Diagnosis typically requires a combination of echocardiography, CT, and MRI to define cyst anatomy and its relation to critical structures. Definitive treatment involves careful surgical excision under cardiopulmonary bypass to prevent rupture and anaphylaxis, accompanied by perioperative anti-helminthic therapy such as albendazole. Early recognition and timely surgical management are crucial to preventing catastrophic complications and achieving favorable outcomes.
