Case description

Heart transplantation is a complex surgical procedure involving the replacement of a patient’s failing heart with a healthy donor heart. The operation typically begins with median sternotomy and the establishment of cardiopulmonary bypass. Once the patient is on full support and cross-clamped, the recipient heart is excised, preserving cuffs of the left atrium, inferior and superior vena cava, and pulmonary artery. The donor heart is then implanted using sequential anastomoses—left atrial cuff, inferior vena cava, superior vena cava, pulmonary artery, and finally the aorta. In some modern approaches, including beating heart or normothermic techniques, the donor heart is implanted without cardioplegic arrest, potentially reducing ischemia-reperfusion injury. After the anastomoses are completed and de-airing is ensured, the aortic cross-clamp is removed, allowing reperfusion of the graft. Hemostasis is secured, and the patient is gradually weaned off bypass while monitoring graft function and hemodynamics.
