Laparoscopic Partial Kidney Excision Due To Renal Tumor In Solitary Kidney Case

Case description

Background: Surgical interventions on a solitary kidney affected by a cancer are one of the most difficult. In this cases laparoscopic partial kidney excision gained popularity. The video presents kidney removal in a solitary kidney with a tumor. Case Report: A 45-year-old gentelman was made a diagnosis of the cancer of the left kidney. Previously he was affected by a Wilms tumor in the right kidney at the age of 2. Kidney excision together with radiotherapy were performed. The renal function of left kidney remained intact, which was proved during the examinations. The CT-scan revealed mass of the size about 17 mm. Because the tumor was suspected to be malignant, the left partial kidney excision was performed using laparoscopic technique. We can see the patient positioning left side up and insertion of 12 mm and 5 mm ports. Adhesions were removed and ultrasound helped to identify the margins of the mass. Hilum exposure showed single vessels, which were clamped. Tumor within the oncological margins was removed and wound was closed. Total warm ischemia duration was 25 minutes. No complications were noted. One-week and 6-month after the surgery the patient was still in a good state and the creatinie level remained normal. Conclusion: Laparoscopic approach can be applied in patients fulfilling all other requirements with solitary kidney and pathological mass.
