Donor Kidney Excision Via Vagina

Case description

Aim: To present a clinical case of excision of the kidney via vagina at living donor. Background: A 47-year-old lady underwent laparoscopic procedure of right kidney removal through a vaginal way. Because the patient had had her uterus excised due to leiomyomata, the incion was made in the posterior vaginal fornix. Intervention: This type of the procedure permitted to reduce warm ischemia time to less than 3 min. There was no intraoperative complication. The donor went home on the POD1 and was in a good state in 4 weeks recovery time. The graft functioned very well, a nadir level of serum creatine was below 0.6mg/dL. The recipient went home on POD7 and was controlled regulary in a 12-week period. Conclusions: Transvaginal excision is a valuable option for a living donor kidney removal. Further studies is necessary to specify the infection risk for the recipient and proper qualification requirements. To spread the technique, follow-up results are needed . Transvaginal extraction can be an introduction to a Natural Orifice Transluminal Endoscopic Surgery (NOTES). It is a step forward in the laparoscopic surgery. Authors: Isabel C.
