Case description

General anesthesia with single-lung ventilation was always considered a condition for thoracoscopic major pulmonary resections. However ,non-intubated thoracoscopic lobectomy (with spontaneous ventilation) has been reported recently by using conventional video-assisted thoracoscopic surgery (VATS), epidural anesthesia and vagus blockade. Here we present the technique minimizing the invasiviness of the procedure even more: single incision VATS approach with no need for epidural or vagus blockade in a non-intubated patient.To our knowledge this procedure represents the less invasive approach to lobectomy.
A 86 year old woman with a 3.5 cm adenocarcinoma was proposed for a non intubated uniportal video-assisted thoracoscopic (VATS) surgery. The total surgical time was 80 minutes. The postoperative course was excellent and the patient was discharged home on the third postoperative day with excellent recovery and no pain.
