Case description

The film presents surgery of the right ear, which is performed for chronic otitis media. The key step is posterior tympanotomy which allows for excellent access to the tympanic cavity by the mastoid. This way the inflammatory tissue like granulation or cholesteatoma can be removed completely, thus reducing the risk of recurrence. An endoscope inserted through the posterior tympanotomy improves the purification of the tympanic cavity by assessment of various recesses of the tympanic cavity. Posterior tympanotomy also allows to perform ossiculoplasty and myringoplasty, the effects of which can be further evaluated by the endoscope. Parallel access through the external auditory canal gives an additional pointof view during myringoplasty. Canal wall up tympanoplasty with posterior tympanotomy is the "gold standard" for the treatment of chronic otitis media, especially in cholesteatoma. The present procedure is carried out by Professor Kazimierz Niemczyk, Head of Department of Otolaryngology, Medical University of Warsaw. The material was recorded during the annual International Course of Microsurgery of the Middle Ear, which was organized already the thirteenth time under the chairmanship of Professor Niemczyk, in Warsaw in November 2012.
