Case description

This video presents the harvesting of the vascularized iliac crest bone graft, done by Professor Shekhar Kumta, The Chinese University of Hong Kong. The pedicled iliac crest bone graft is based on the Deep Circumflex Iliac artery. This is a branch of the External Iliac artery and it breaches of the external iliac artery just above the inguinal ligament. The vascularized iliac crest one graft is a head-preserving procedure. It is best suited to the pre-collapse phase of the disease. FICAT Stage II and Stage III with a crescent sign but without collapse are the patients who do well. The x-rays are shown and explained. Patients examination shows the lost of internal rotation in his hip joint. CT and MRI can help to evaluate the scale of the necrosis. Finally, a surgery is explained and presented.
