During a filtration surgery, the sleeve of 100 micron Fugo blade tip came off and stayed in the track line. Later the proximal tip of this compulsory seton eroded through the conjunctiva and caused leakage. The sleeve/seton was pulled out. When reviewing the surgery, it was felt that we could have done this in a more sophisticated and atraumatic manner.
