We present not everyday surgery case of VA/PICA aneurysm clipping approached through the left retrosigmoid craniotomy. Patient in prone Concorde position with head slightly rotated to the left. After successful clipping the exclusion of the aneurysm and patency of both: VA and PICA was checked with ICG-videoangiography.
