Case description

This video was part of the Video Loop during the SAGES 2020 Virtual Meeting. Topic: Foregut Laparoscopic Sub-total Gastrectomy for Management of Bleeding Polyps. Rachel Groening, Michael Latzko. Advent Health Sebring. Menetriers Disease (MD) is a type of hypertrophic gastropathy characterized by hypertrophy of the gastric mucosa with diminished parietal and chief cells on histology. Often MD can produce intractable bleeding which is difficult to control. In refractory cases surgical gastrectomy may be required. We present a case involving a 58-year-old female with a two-year history recalcitrant anemia, due to recurrent upper GI bleeding from enlarging polyps demonstrated on endoscopy. We demonstrate a laparoscopic subtotal gastrectomy with a Roux-en-Y gastrojejunostomy for the management of this patient's multiple bleeding polyps. Histological evaluation and EGFR staining of the specimen revealed characteristics specific for MD.
