Caterpillar (Moynihan’s) Hump Anomaly Encountered During Laparoscopic Cholecystectomy

Case description

While performing laparoscopic cholecystectomy in a obese male of BMI 40, after obtaining critical view of safety, a large pulsating vessel presumed to be cystic arteries with overlying node was found. But meticulous dissection revealed it to be caterpillar hump of right hepatic artery and a very short cystic artery arising from it. Cystic artery was dissected and clipped very close to GB wall and safe cholecystectomy was performed. Failure to identify this anomaly can lead to torrential life threatening hemorrhage during laparoscopic cholecystectomy.
