Case description

On doing Lap Chole for patients with previous upper abdominal surgery, it is better to avoid the area of the previous surgery during insertion of your Veress needle and your first port. If your insert your Veress needle with wrongly in the umbilicus in patient with previous PUH please insert your first port away from this area as the Rt Subcostal site and use a 5mm scope to evaluate your Veress needle entry and to do adhesiolysis after insertion of another port as the subxiphoid one. Lastly, discuss with your camera man and assistant where to insert your camera port away from the area of your previous operation where the Mesh was implanted and the Prolene sutures are visible. Also, don’t accuse the previous surgeon that he didn’t protect the visceral with peritoneal closure with absorbable sutures, as I was that person who did this surgery 9 years ago. But if this is true, what is beyond these extensive adhesions in this area? Noting that the repair was open with onlay mesh repair.
