A surgical video where a left lumbar hernia was operated in the pre-peritoneal space by a combined transabdominal and prepereritoneal approach in a TEP like procedure, where the 10mm trocar used for optics and mesh insertion was placed under direct view in the preperitoneal space, hernia hole was fully dissected by minilaparoscopy, preperitoneal lipoma reduced and a prolene mesh placed overlaping hernia hole, without tackers or any other kind of mesh fixation. At the end of the procedure a laparoscopic view shows perfect mesh placement.
