Case description

 Esophageal perforation after endoscopic esophagus dilation in a patient with achalasia is a serious complication inherent in the procedure, occurring in 2--6% of cases. Reports have been made on the treatment of this injury by using the laparoscopic technique with closure of the perforation, either with or without the contralateral Heller's myotomy and an antireflux procedure. However, the treatment of this severe injury using Thal's esophagogastroplasty with Dor's anterior fundoplication (Thal-Dor) by laparoscopic approach has not been reported earlier. In our study, we have reported the case of a patient with idiopathic achalasia who suffered a rupture of approximately 6 cm in the distal esophagus during a procedure of endoscopic balloon dilation. The injury was repaired using a Thal-Dor procedure performed entirely by laparoscopy. Nowadays, minimally invasive surgery is being increasingly used in general surgery. The resources available are safe and efficient, as a result of which the complex procedures can be performed successfully even in emergency cases.
