47-years old female Years history of chest pain, regurgitation of undigested food, bad breath, A full blood count showed a mild, microcytic, anemia Esophagogastroduodenoscopy revealed a giant midd esophageal diverticulum with necrotic areas and food putrefaction Surgical treatment: thoracoscopic excision (Dr. Mocan Lucian 3-rd Surgery Clinic Cluj Napoca Romania) Intraoperative: a very large, necrotic diverticulum with inflammatory chronic adhesions in left hilum of the lung and aorta The postoperative outcome was uneventful the patient was discharged in the 5-th postoperative day
