Case description

In this patient as a result of prolonged negative pressure in the middle ear due to eustachian tube dysfunction, the tympanic membrane has been retracted medially and is resting against the head of the stapes. Note that the long process of the incus has been resorbed. The stapedial tendon can be seen attaching to the head of the stapes [11 o'clock position].
