Case description

The posterior inferior quadrant of this tympanic membrane has lost its supporting fibrous middle layer and as a result has been retracted medially by negative pressure in the middle ear. This is referred to as atelectasis. As we will see in a subsequent photograph if this patient performed a Valsalva's manoeuvre auto inflating the middle ear, this thinned dimeric tympanic membrane will herniate outwards. This is referred to as a "pulsion hernia".
