Case description

Critical issue during the insertion of a IOL into a highly myopic patient (due to axial elongation) is keeping the anteriot chamber formed all the time. Risk of retinal detachment should be avoided - it is caused by collapse of the anterior chamber causes antero-posterior movement of the vitreous base. This clip presents removal of the end of cortex removal, inflation of the capsular bag with viscoelastic via the side-port while the irrigation is maintained via the main incision. Once the IOL is in the capsular bag, the viscoelastic aspiration is performed via the probe and then surgeon injects balanced salt solution via a blunt canula into the anterior chamber via the paracentesis while the probe is withdrawn. It allowes to avoid the AC collapse throughout the entire surgery.
