Case description

Uterine sacculation during pregnancy is a rare complication of retroverted uterus. Uterine position has no effect on fertility. A tipped uterus will usually right itself during the 10th to 12th week of pregnancy. On rare occasions the uterus fails to become anteverted, and the pregnancy continues to expand the retroverted uterus and develop uterine sacculation. This is an aneurysmal-like dilatation of the most superior portion of the uterine wall that permits the uterus to enlarge with the consequence of major anatomic distortion. Vaginal delivery is difficult, and even cesarean section can be difficult. A thin uterine wall, as a result of uterine sacculation may also induce uterine rupture.
In this case the uterus was opened by section in upper segment to avoid amputation of cervix, and perforation of posterior wall was repaired.
