Case description

"White bile in gallbladder hydrops" is a relatively rare finding and usually indicates long-standing obstruction of the cystic duct. In this condition, bile becomes trapped within the gallbladder and is gradually absorbed. After the bile pigments (bilirubin and biliverdin) are absorbed, the remaining fluid takes on a white, mucoid, or milky appearance. This phenomenon is known as "white bile" or "milk of calcium bile." The formation of white bile is usually associated with chronic inflammation or infection in the gallbladder. Obstruction can be caused by gallstones, tumors, or other factors. Gallbladder hydrops, meaning distension of the gallbladder due to fluid accumulation, is often associated with white bile. Treatment for gallbladder hydrops with white bile usually involves surgical removal of the gallbladder. This is done to prevent complications such as gallbladder rupture, cholecystitis, or progression to empyema.
