60 years old female presented with severe dysphagia, diagnosis proved to be primary achalasia. Laparoscopic Heller Myotomy done, then Hiatoplasty for large Hiatal H. Done and posterior partial fundoplication (Toupet procedure) as antireflux procedure done.
