This video vignette presents the case of a 67-year-old female with adenocarcinoma of the proximal descending colon who underwent a laparoscopic left extended hemicolectomy with derotation of the right colon and colorectal anastomosis. The video presents the technical difficulties encountered during mobilization of the left colon to procure a safe approach to the splenic flexure. It further showcases a complete laparoscopic mesocolic excision with high vessel ligation to achieve a safe oncologic excision. Finally, it presents the rare instance when a complete laparoscopic derotation of the ascending colon is used to provide length for a tension free primary colorectal anastomosis.
