Case description

VATS thymectomy is a minimally invasive surgical procedure using video-assisted thoracoscopic surgery to remove the thymus gland, primarily for conditions like myasthenia gravis (MG) or thymic tumors. This technique offers advantages over traditional open surgery, including less pain, a shorter hospital stay, better lung function, and improved cosmetic outcomes. It can be performed via a unilateral or bilateral approach, often with a right-sided preference for its good view of venous anatomy, though left-sided and combined cervical approaches are also used.  

Purpose of VATS Thymectomy

Myasthenia Gravis (MG): To remove the thymus gland, which can be a source of the antibodies that cause the muscle weakness in MG, and to remove any ectopic thymic tissue.  

Thymic Tumors: To remove benign or malignant tumors of the thymus.

How the Procedure is Performed

1. Patient Positioning: The patient is generally placed in a supine or 30-degree lateral decubitus position.  

2. Incision and Port Placement: Small incisions (ports) are made in the chest, often along the submammary fold.  

3. Visualization:A  thoracoscope (camera) is inserted through one of the ports to provide a magnified view of the surgical area on a monitor.  

4. Dissection and Removal: Surgical instruments are inserted through other ports to carefully dissect and remove the thymus gland.  

Key Aspects and Approaches

Unilateral VATS: A standard approach through one side of the chest, often the right for better venous anatomy visualization and space.  

Bilateral VATS: A more comprehensive approach, sometimes including a cervical (neck) incision, to ensure complete removal of the thymus and mediastinal fat, particularly in MG patients.
