Radical surgical excision is potentially curative, and intensive follow-up is recommended.  Methods: A 22-years-old Caucasian woman suffering from SPN of the pancreatic tail (45 × 47 mm in diameter) is presented. The patient underwent robotic spleen-preserving distal pancreatectomy. Intraoperative ultrasound (US) allowed to clearly identify the splenic vessels.  Results: Operating time was 240 mins. No bleeding or complication happend during the operation. The distal pancreatic duct was identified and closed. Postoperative course, following Enhanced Recovery After Surgery (ERAS) protocol, was uneventful.
