Case description

А 72 years old female was admitted to the clinic with emergency condition of painless, rigid tumour mass, located in the right inguinal channel region. The complaints dated from 4 hours. Several episodes of vomiting. The patient had similar episodes of incarceration from the past, but they resolved spontaneously. History data for previous conventional surgery – lower median laparotomy for total hysterectomy was established. Local findings: in right inguinal channel - rigid tumour mass, with regular shape, painless, without ability for repositioning. Preoperatively sonography was performed: sonographic data for liquid into the hernial sac and small bowel in it. Patient was scheduled for emergency operation, choosing TAPP technique. The first working trocar was placed using Hasson open technique. Exploration of the abdominal cavity was performed. The incarcerated small bowel loop was retracted into the abdominal cavity and was revised for any ischemic lesions. The good vitality of the segment was approved. The procedure continued with incision of the parietal peritoneum, retraction and separation of the hernial sac. Placement of 15/10 mm polypropylene mesh was performed. Peritoneum was sutured using Vylog 3/0. The patient was discharged on the 2nd postoperative day.
