Case description

Ex vivo lung perfusion (EVLP) is a cutting-edge technique used to evaluate and rehabilitate donor lungs prior to transplantation. However, one of the challenges associated with EVLP is the development of lung edema during the perfusion process. EVLP-induced lung edema occurs due to increased vascular permeability and impaired fluid clearance, often triggered by inflammatory responses and ischemia-reperfusion injury inherent in the ex vivo environment. This edema can compromise lung compliance and gas exchange, ultimately affecting the quality and transplantability of the organ. Understanding the mechanisms behind EVLP-related edema is critical for optimizing perfusion protocols and improving post-transplant outcomes.
