Masive Subretinal Hemorrage and Combined PDR Retinal Detachment

Case description

We present the case of a 45-year-old man with bilateral PDR and a total combined tractional and rhegmatogenous left eye retinal detachment, associated with a massive subretinal hemorrhage. Using the Constellation® VR platform (Alcon, Forth Worth, Texas, USA), a monomanual 25G pars plana vitrectomy technique was performed. Initially, a delicate segmentation, peeling and extraction of the fibrotic tissue above the retina was done without the needof a second-hand instrument besides the lightning prove. After the release of the main retinal tractions and the identification of a superonasal equatorial retinal hole, perfluorocarbon liquid was injected to stabilize the retina. Using a 25G silicone tip canula, subretinal rTPA was injected through the retinal hole allowing a partial thrombolysis of the blood and hence permitting the aspiration of a great amount of the massive subretina hemorrhage. The eye was finally filled with 5000ctk silicon oil after a complete panretinal photocoatulation.
