Case description

This is an illustration of the procedure performed on a 37-year-old lady with a Body Mass Index achieving 57. Despite open gastric bypass surgery in 1994, the patient was not able to lose her weight. Panendoscopy revealed a large pouch and, because of that, the patient was transported to the Operatory Room for laparoscopic bypass examination. The stomach was mobilized, and with use of EndoGIA, partitioned slightly above and below the former staple lines. Then, the fundus was shared and excised. Once the jejunum separated from the gastrojejunostomy with an EndoGIA load, the 2 layers gastrojejunostomy was performed. On the first postoperative day, ultrasound indicated a functional, little pouch. No leakage was found. 6 months later the patient presented with the 81 pounds loss. Laparoscopic examination of open Roux-En-Y gastric bypass is considered to be a secure and practicable procedure, which can be applied even in patients after open weight-loss surgery.
